Vacation and Time Off Requests

Step 1. Review your existing schedule and make sure we don’t have anything scheduled.

Step 2. Select Month View

Step 3. You can scroll into previous and future months by clicking on Previous Month or Next Month on the bottom of this Monthly Calendar page

Step 4. If you have visits already scheduled during your Time-Off, please notify the office at [email protected]

Step 5. To submit a time-off request, click on Profile

Step 6. Scroll to the bottom of the page to the Time Off Notes field. Enter the dates you’ll be gone and any additional information that goes with it. Click Submit.
